If your client wants the index provided in a special format you can define those styles in the User Defined Codes tab. There are presets for RTF, RTF without headers/footers, RTF indented with spaces, RTF indented with tabs, Chicago Press, ANSI/NISO, HTML, and Text (ASCII).
